How could a baby who's up for adoption have false/mixed up papers?

I'm doing some creative writing and doing a scene where a baby is just born, he's put into the baby nursery at the hospital, then child services (or whoever) comes to pick him up. EXCEPT that I need the hospital to somehow screw up the papers of the baby, giving him a false identity sort of.





So I'm asking, for all of you mothers or fathers, how could I make them screw the papers up? I'm thinking along the lines of ditzy nursing intern fudging something...thanks.How could a baby who's up for adoption have false/mixed up papers?
There have been a number of cases, in real life, where babies were switched in hospitals.





These babies grew up in different families, and their identities were revealed years later, by doing DNA tests.





There have also been a number of cases where babies were born to mothers who used sperm donation techniques, in their doctor's offices. These women thought they were using their husband's sperm, but it was another man's sperm sample.How could a baby who's up for adoption have false/mixed up papers?
Well maybe the mom putting the baby up for adoption said she didn't want to see the baby because, it was to hard. Then the other mom had complications like maybe very high blood pressure so they didn't let the mom see the baby before they took it out of the room because, they were taking care of her. Maybe both the babies were born at the same time and it was very busy in OB that day and the same nurse was documenting on both babies charts and accidentally put the wrong charts on the wrong bed. You might want to look into Pregnancy situations where they might put you to sleep or something when you give birth so you can make the situation sound like it could be true or ask on here if that has happened to anyone and if so why.

Well it is possible that the parents bribed the doctor/nurse to write up false papers on the baby.


Or maybe the doctor/nurse was too busy with other duties and didn't have the time to write up accurate papers and the time ran out so the nurse/doctor wrote up false papers really quickly.


Maybe the mom was hiding the baby from her family or her boyfriend, like say the guy wouldn't have let her put the baby up for adoption so she snuck into the office and screwed up her child's papers so the father wouldn't take the baby.
Well, in my real life, when I was put in the nursery, they wrapped me in a blue blanket instead of pink. The birth date and name were also changed so that no one could steal me. They were worried that my original mother would change her mind. It was quite a scandal. Maybe you could work something in of that nature.
